•The PA/PoA/CP design document (PA-DD, PoA-DD/CP-DD)
•The qualification proofs of the project owner and/or the developer (for example, the copy of business license)
•The letters of approval from the DNA of the Parties of the activity participants (Confirmation on UNFCCC website)
•Location description
•Related records of project local stockholders’ opinions
•Evidence for the prior CDM consideration and project additionality
•Clarification regarding the overlap with other GHG mechanism
•The forms related with A6.4 sustainable development tool
Applicant for CEC A6.4 verification/certification service should complete the “Application Form for Validation, Verification and Certification” and provide relevant documents as below:
•Evidences of project registration by the SB
•Final version of the project design document and related appendixes
•Final version of the validation report
•The resisted PDD and validation report which used for post-registration change, if applicable
•The preliminary assessment report regarding actual reversal, if applicable
•The forms related with A6.4 sustainable development tool which indicated the outcome
•Evidence for PA/PoA/CP operation status/implementation 
•The latest monitoring report of implementation (if applicable)
•The quality control (QC) and quality assurance (QA) procedure 
•Other documents such as the process flow chart, equipment technical manuals, etc.
